Relapsing polychondritis is a rare, generalized autoimmune disorder affecting primarily cartilaginous and proteoglycan-rich structures. Central nervous system (CNS) involvement is a rare complication of relapsing polychondritis. A 49-year-old man with past history of relapsing auricular chondritis was presented with generalized seizure and behavioral change. Laboratory findings suggested aseptic meningitis while brain images showed subarachnoid hemorrhage. The patient did not respond to antibiotics and antiviral agents. Meanwhile episcleritis that the patient developed during hospitalization suggested that CNS may be involved in relapsing polychondritis. His symptoms all improved after using corticosteroids. We conclude that meningoencephalitis and subarachnoid hemorrhage can be a clue in diagnosing relapsing polychondritis.
