당뇨를 가진 급성 뇌경색 환자의 초기 혈당변동과 3개월 이내 심혈관계 사망의 연관성

Abstract

Background:

Blood glucose fluctuation is linked to mortality in critically or non-critically ill patients, as well as cardiovascular events after acute myocardial infarction. However, its association with poststroke outcome is largely unknown. The aim of this study was to determine whether initial glucose fluctuation is related to cardiovascular mortality in patients with acute ischemic stroke.

Methods:

We registered consecutive patients with acute ischemic stroke in Soonchunhyang University Hospital between Mar 2005 and Feb 2015. Six hundred and thirty-nine diabetic patients within 72 hours from stroke onset were included. Serum glucose levels were checked 4 times per day during the first 3 hospital days. J-indices were calculated for glycemic variability in all patients, and divided into 4 groups by quartiles. Cardiovascular death and modified Rankin scale at 3 months were prospectively recorded. Multivariable logistic regression analyses were performed after adjusting covariates for outcome variables.

Results:

Cardiovascular deaths at 3 months were identified in 57 patients (8.9%): 11 (7.1%), 8 (5.1%), 13 (8.0%) and 25 (15.2%) in each of the J-index quartiles, respectively. Multivariable logistic regression analysis indicated that the incidence of cardiovascular deaths were significantly associated with the group of highest J-index (odds ratio 3.38; 95% confidence interval 1.15-12.04; P=.04).

Conclusion:

Fluctuation of serum glucose during the first 3 days was associated with cardiovascular death at 3 months after acute ischemic stroke. Further studies are required to determine the pathophysiologic correlation between glycemic variation and cardiovascular deaths.

서 론

제2형 당뇨병은 관상동맥질환, 뇌졸중, 말초동맥 질환 등 큰동맥질환 합병증을 흔히 동반하며[1], 혈당의 불충분한 조절로 인한 고혈당은 심혈관계질환의 발생 및 사망률을 증가시킨다[2]. 한편, 당뇨병의 치료가 지속될수록 환자의 혈당 수치는 점점 일중 변동량이 커지게 되는데, 혈당 수치의 변동이 커질수록 심혈관계 질환의 위험성이 증가되며[3,4], 중환자 및 비중환자 모두에서 입원 중 사망이 증가하는 것으로 알려져 있다[5-9]. 혈당변동이 심혈관질환이나 사망의 위험을 증가시키는 기전은 산화스트레스(oxidative stress)의 활성이나 세포자멸사(apoptosis)가 활성화되고, 혈관수축이나 미세혈관 혈전, 그리고 염증반응이 증가하는 것에 기인하는 것으로 것으로 여겨지고 있다[10-12].
이전 연구에서 급성 뇌경색 발생 전후의 고혈당은 뇌경색 후 재발 및 사망과 관련이 있었다. 당화혈색소(Hemoglobin A1c)는 급성 뇌경색 후 1년 이내의 뇌경색 재발과 관련이 있으며[13], 급성 뇌경색 초기 고혈당은 뇌경색 후 사망률을 높였다[14]. 하지만 뇌졸중 급성기의 혈당변동과 뇌졸중 후 예후와의 관련성에 대해서는 아직 알려진 바가 없다. 혈당변동은 만성적으로 지속되는 고혈당보다 더 심한 대사성 변화를 초래하여, 세포사를 촉진시키며[15], oxidative stress 및 혈중 사이토카인 농도를 증가시킨다는 연구 결과가 있다[10,12,16].
이에 본 연구는 당뇨병을 가진 중증 급성 뇌경색 환자에서 초기 혈당의 변동이 뇌경색 후 첫 3개월 동안 심혈관계 질환에 의한 사망과 연관이 있는지를 알아보고자 하였다.

대상 및 방법

대상환자

본 연구는 2005년 3월부터 2015년 2월까지 순천향대학교 서울병원에 입원한 발생 후 3일 이내의 급성 뇌경색 또는 일과성허혈발작 환자를 대상으로 하였다. 레지스트리는 내원한 환자를 빠짐없이 웹기반으로 전향적으로 등록하였으며, 환자의 임상양상, 위험인자, 검사 및 영상소견, 치료종류 및 퇴원 후 예후에 대한 정보를 포함하고 있다[17]. 입원 중 확산강조 자기공명영상에 병변이 보이지 않는 일과성허혈발작 환자, 악성종양이나 다른 대사성 중증 질환으로 여명이 3개월 미만으로 추정된 환자는 제외되었다. 당뇨병의 진단 기준은 1) 입원 전 혈당강하제를 복용하던 환자, 또는 2) 입원 중 시행한 당화혈색소 수치가 6.5 이상인 환자, 또는 3) 뇌경색 안정 후 공복혈당이 126 mg/dL 이상이거나 식후혈당이 200 mg/dL 이상인 환자로 정의하였다[18]. 본 연구의 디자인은 순천향대학교 서울병원 윤리심의위원회의 승인을 받았다.

혈당변동의 측정 및 J-index

입원 첫 3일간 혈당 수치 자료 수집은 후향적 의무기록 조사를 통해 이루어졌다. 모든 환자에서 매일 하루 4회(오전 6시, 오전 9시, 오후 2시, 오후 8시) 혈당이 측정되었다. 일반적으로 혈당 수치는 정규분포를 하지 않기 때문에 혈당변동은 표준편차로 제시되었을 때 단점이 있고, 심한 저혈당을 충분히 반영하지 못한다[19]. 이에 본 연구에서는 간헐적 혈당 수치의 평가에 가장 적합한 J-index를 이용하였으며, 이는 하루 동안의 혈당 변동 및 평균 정도를 나타내는 감한 지표로서 심한 저혈당이나 고혈당까지 반영하여 하루 동안의 혈당 변동성을 나타낸다[20]. 일반적으로 건강한 성인에서 J-index는 10 정도를 보이며, 7.5 미만의 낮은 수치는 저혈당의 위험을 30 이상은 조절되지 않는 혈당조절 상태를 의미한다[20].

본 연구에서 혈당변동은 3일동안 측정된 12회의 혈당 수치로 계산되었으며, 측정된 혈당이 하루 3회 미만 또는 총 8회 미만으로 일중 혈당변동을 충분히 반영하기 어렵거나, J-index 계산이 불가능한 경우 분석에서 제외하였다.

사망연관 변수

웹기반 레지스트리에 등록된 변수 중 기존 연구에서 뇌경색 후 사망과 연관이 있는 인자를 수집하였다. 주요 변수는 나이, 성별, 과거 뇌졸중병력, 초기 National Institute of Health Stroke Sacle (NIHSS), 내원시 수축기/이완기 혈압, 혈중 콜레스테롤 수치, 흡연유무, 심방세동 유무, 백혈구수치, 첫 혈당수치, 당화혈색소수치를 포함하였고, 뇌경색의 중증도는 미국립보건원뇌졸중척도(NIHSS)에 따라 경증(0-4), 중등도(5-15), 중증(16-)으로 분류하였다[21,22].

사망 및 예후 자료수집

연구기간 동안 급성 뇌경색 환자의 입원 시 모든 환자에게 향후 심혈관질환 및 사망에 대한 예후 정보를 제공하기로 서명한 동의서를 받았다[17]. 웹기만 레지스트리에 등록된 모든 환자의 예후자료는 전향적으로 수집되었으며, 퇴원 후 3개월 심혈관계 사망의 평가 및 기능적 상태(modified Rankin Scale, mRS)는 발생 3개월째 병원 방문 기록 또는 뇌졸중 코디네이터와의 전화 상담을 통해 확인되었다. 사망의 원인이 사고, 악성종양, 감염 및 기타 내과질환의 뚜렷한 원인이 있는 경우는 심혈관계 사망에서 제외하였다.

통계분석방법

결과에서 범주형 변수는 빈도, 연속형 변수는 평균/표준변차로 제시하였고, J-index의 각 사분위 집단의 임상소견 및 뇌경색 후 예후을 비교하였다. 또한, 3개월 심혈관계 사망 유무에 따라 두 군의 기본 임상양상, 검사소견, 그리고 심혈관계 사망에 영향을 줄 수 있는 인자를 비교하였으며, T-검정 및 카이제곱 검정을 통해 단변량 분석하였다. 3개월 심혈관계 사망 유무와 연관된 인자는 로지스틱 회귀분석을 통해 조사하였는데, 단변량 분석에서 유의수준 P값이 0.2 이하 및 임상적으로 주요 위험요인으로 생각되는 인자들을 공변량으로 포함시켰다. 로지스틱 회귀분석 결과에서 유의수준 P값이 0.05 이하인 경우 통계적으로 의미가 있는 것으로 하였으며, 모든 통계분석은 SPSS 18.0 (SPSS Inc., Chicago, IL, USA)을 사용하였다.

결 과

대상환자 및 임상양상

2005년 3월부터 2015년 2월까지 순천향대학교 서울병원에 입원한 3일 이내의 뇌경색으로 확산강조자기공명영상에서 급성 병변이 확인된 총 1,629명 환자 중 678명(41.6%)이 당뇨병을 가지고 있었다. 이 중 악성종양이나 내과적 중증질환으로 기대여명이 3개월 이하로 기록된 28명을 제외하였다. 그리고, 입원 후 첫 3일간 혈당검사가 충분히 시행되지 않았던 11명을 제외한 총 639명을 분석 대상으로 하였다. 환자의 평균 나이는 68.0±11.4세였고, 남자는 380명(59.5%)이었다.

J-index 사분위군에 따른 임상양상

J-index 각 사분위군에 따른 임상양상, 검사소견은 Table 1에 기술되어 있다. 단변량 분석에서 각 군에서 나이, 성별의 차이는 없었으며, 초기 수축기/이완기 혈압, 지질 수치, 백혈구수치도 비슷하였다. 신경학적 결손의 차이는 NIHSS 4점 이하, 4-15점, 16점 이상으로 구분하였을 때 J-index 사분위군이 높아질수록 NIHSS 16점 이상의 비율이 높았다(P=0.01). 또한, 사분위군이 높아질 수록 내원 첫 혈당수치(P<0.01)와 당화혈색소수치(P<0.01)의 평균이 높았다. 반면, 심방세동의 분율은 J-index 사분위군이 높아질수록 적게 나타났다(P=0.05).
뇌경색 후 3개월째 사망은 J-index의 각 사분위군에서 7.1%, 5.1%, 8.0%, 15.2%였으며(P=0.01), 불량한 기능적 상태인 3점 이상의 mRS 분율은 39.9%, 40.9%, 36.1%, 51.6%로 나타나(P=0.04), 모두 가장 높은 사분위군에서 예후가 좋지 않았다.

3개월 심혈관계 사망 유무에 따른 임상양상

전체 환자 중 3개월째 사망은 57명(8.9%)에서 발생하였다. 환자를 3개월 심혈관계 사망 유무에 따라 비교하였을 때, 심혈관계 사망 환자군에서 나이가 많았으며(73.7±10.6 vs. 67.5±11.3, P=<0.01), 과거 뇌졸중 병력이 더 많았다(18.0% vs. 26.3%, P=0.15). 심혈관계 사망 환자군에서 입원 초기 NIHSS 4점 이하의 환자군이 적었고(54.1% vs. 21.0%, P=<0.01), 16점 이상의 환자가 많았으며(7.7% vs. 43.9%, P=<0.01), 심방세동의 분율이 높았다(16.2% vs. 43.9%, P=<0.01). 백혈구수치는 사망환자군에서 높았으나(9.4±7.8 vs. 14.3±16.6, P=0.04), 흡연은 비사망군에서 더 많았다(31.9% vs. 18.2%, P=0.04). 수축기/이완기 혈압, 총콜레스테롤은 양군에서 차이가 없었다(Table 2).
입원 첫 혈당 수치는 사망군에서 높았으나(160.7±73.6 vs. 190.1±82.7, P=0.02), 당화혈색소 수치는 심혈관계 사망과 비사망군 사이에 차이가 없었다(7.9±1.9 vs. 7.6±1.6, P=0.35). J-index의 분포는 두 군에서 차이가 있어, 사망환자군에서 J-index 고위 사분위군의 비율이 높았다(P=0.01).

급성 뇌경색 발생 3개월 후 심혈관계 사망 예측

로지스틱 회귀분석에서 J-index의 가장 높은 사분위군은 유의하게 뇌경색 발생 3개월 후 심혈관계 사망과 연관되었다(odds ratio [OR] 3.38, 95% confidence interval [CI] 1.15-12.04, P=0.04). 그 외 심방세동(OR 3.28, 95% CI 1.32-8.11, P=0.01), 과거 뇌졸중병력(OR 3.00, 95% CI 1.29-7.02, P=0.01), NIHSS 5점 이상 및 16점 이상에서도 3개월 심혈관계 사망이 높았다(OR 6.29, 95% CI 2.35-16.86, P=<0.01; OR 13.86, 95% CI 4.72-40.69, P=<0.01). 반면에, 입원 첫 혈당 수치는 예측 모델의 변수로서 유의하지 않았다(OR 1.00, 95% CI 0.99-1.01, P=0.94) (Table 3).
사망을 예측하는 가장 강력한 변수인 NIHSS를 통제하기 위해, 각 NIHSS군에서 층화분석(stratification analysis)을 실시하고, 각각의 로지스틱 회귀분석 공변량 변수는 동일하게 하였다. NIHSS 4점 이하 환자만 대상으로 하였을 때 J-index의 가장 높은 사분위는 3개월째 사망과 OR 3.20(95% CI 0.97-81.84, P=0.05), 5-15점 환자에서는 OR 5.91(95% CI 0.58-61.71, P=0.13), 16점 이상 환자에서는 OR 17.90(95% CI 1.35-335.03, P=0.02)로 연관이 있었다.

고 찰

본 연구는 약 10년동안 단일 병원에서 빠짐없이 등록된 당뇨병을 가진 급성 뇌경색 환자를 대상으로, 입원 첫 3일간 혈당변동이 뇌졸중 3개월 후 심혈관계 사망과 유의한 관련이 있음을 보여주었다. 현재까지 혈당 변동성에 따른 관상동맥질환의 예후와 관련된 연구는 있었으나[4], 아직 급성 뇌경색 환자에서 혈당변동의 빈도의 기술이나 예후와의 연관성을 밝힌 이전 연구는 찾아보기 어렵다.
이전 연구들에서 급성기 뇌경색 초기 혈당 수치는 뇌경색 후 출혈성 변환의 증가 및 사망 연관이 있었다[14,23]. 하지만, 본 연구에서는 초기 1회 측정된 혈당 수치보다 첫 3일간의 혈당 변동이 뇌경색 발생 3개월후 심혈관계 사망과 더 연관된 것으로 나타났는데, 이는 혈당변동이 지속적인 고혈당보다 산화스트레스를 더 유발시켜 사망의 증가와 연관될 수 있고[10], 실제 스트레스 고혈당은 패혈증과 같은 중환자에서 그다지 위험하지 않다는[24] 이전 연구결과와 유사하다. 하지만 급성기간 지속적으로 측정된 고혈당은 뇌경색 병변의 증가 및 불량한 예후와 관련이 있다는 보고도 있어[25], 향후 뇌경색 후 심혈관계 질환 발생 및 사망을 예측하기 위한 초기 혈당변동과 지속적인 고혈당의 상관관계는 좀 더 잘 짜여진 디자인의 연구가 필요할 것으로 보인다.
초기 신경학적 결손 정도는 많은 연구에서 뇌경색 후 사망과 매우 밀접한 관련이 있어서[26,27], 본 연구에서는 이러한 강력한 교란변수인 NIHSS를 보정하기 위해 층화분석을 추가로 시행하였다. 비록 중등도(NIHSS 5-15) 신경학적 결손을 가진 층화군에서는 유의한 결과를 보이지 않았지만, 대체적으로 혈당변동이 가장 심한 사분위군은 각 층화군에서 사망과 관련이 있는 경향을 보였다. 특히 경증(NIHSS 0-4)의 뇌경색 환자군에서 심한 혈당변동은 심혈관계 사망을 약 3배 증가시킨 것으로 보여, 혈당변동은 뇌경색의 중증도와 독립적으로 사망과 연관되어 있음을 보여주었다. 또한, 고령, 남자, 심방세동은 분석 대상수가 충분히 많지 않은 본 연구의 특성상 통계적으로 모두 유의하지는 않았지만 뇌경색 후 심혈관계 사망과 연관된 경향을 보였고, 이는 이전의 기존 연구들의 결과와 거의 일치한다[28-31].
언급하였듯이, 혈당 변동성에 따른 심혈관계 질환 발생 기전은 아직 확실하지 않지만, 지속적으로 발생하는 일시적인 고혈당에 의한 산화스트레스가 주된 요인으로 생각된다[32]. 고혈당에 의한 산화스트레스는 백혈구-내피세포 간 상호작용을 유도하고, 지질단백질(lipoprotein), 아포지질단백질(apolipoprotein), 응고인자(clotting factor) 같은 신체 단백질의 당화 과정을 일으킨다[33-35]. 이러한 탈수 및 산화 반응이 반복되면서 최종당화산물(advanced glycoslation end products, AGE)이 형성되고, 이는 특히 혈관벽에서 콜라겐과 세포외 매트릭스 단백질의 가교반응(cross-linking)을 유도하여 저밀도 콜레스테롤(LDL) 분자 형성을 촉진한다[36]. AGE로 인하여 변성된 LDL(AGE-modified LDL)은 반감기가 더 연장되어 LDL 분자들의 산화 과정이 늘어나고, 또한 AGE는 또한 내피 세포 기능에 영향을 미쳐 죽상경화판 형성을 촉진한다[37-40]. 즉 일시적인 고혈당은 단핵구가 내피 세포에 부착하도록 유도하며 내피 세포에 부착된 단핵구는 반복적으로 혈당 변동성을 일으켜 악순환이 되면서 죽상경화판의 형성을 가속화시키는 것으로 보인다[41]. 이러한 일시적인 고혈당은 만성적으로 안정적인 고혈당보다도 더 ICAM-1, VCAMP-1, E-selectin의 생산을 증가시켜 내피 세포의 활성화를 촉진시킨다는 실험 결과가 있다[12,15].
본 연구는 단일 병원에서 시행한 비교적 환자 수가 적은 후향적 연구라는 점에서 일반화 하기 어려운 제한점이 있다. 하지만, 뇌경색 급성기에 혈당변동이 초기 혈당 수치를 비롯한 다른 예후 인자를 보정한 후에도 심혈관계 사망에 영향을 미치는 점을 보여준 최초의 연구로서, 향후 좀 더 많은 환자를 대상으로 하는 전향적 다기관 연구의 필요성을 보여준 데 의미가 있겠다. 둘째, 많은 변수들이 존재하는 급성 뇌경색 시기에 혈당변동 및 사망에 영향을 미치는 다른 변수들을 모두 통제하지 못한 점을 들 수 있다. 실제로 감염 등 입원 중 합병증은 뇌경색 후 예후를 불량하게 하며[42], 이는 혈당변동에도 영향을 미칠 것으로 생각된다. 본 연구에서는 심혈관계 사망에서 기타 합병증 등의 원인에 의한 사망을 전향적으로 배제하였으나, 병원 내 사망이 아닌 경우 전화상의 상담으로 사망원인을 파악하는 데는 한계가 있었을 것으로 보인다.
결론적으로, 본 연구에서 급성 뇌경색 입원 후 초기 3일간의 혈당변동은 3개월 후 사망과 연관이 있었다. 향후 혈당변동에 의한 사망이 심혈관계 질환의 증가에 의해 유발되었는지의 여부, 지속적 고혈당과의 상관성 비교, 초기 혈당의 철저한 조절에 따른 심혈관계 질환 감소 여부에 더 많은 연구가 이루어져야 할 것으로 사료된다.
